SKYWARN Spotter Report Format

Use this structured format when reporting severe weather to NWS via SKYWARN net. Complete reports help NWS make better warning decisions.

  1. Your callsign and name
  2. Location — lat/lon preferred, or distance and direction from nearest town
  3. Time of observation (UTC)
  4. What you observed — tornado, wall cloud, funnel cloud, hail, flooding, wind damage, etc.
  5. Measurement — hail size (use reference objects: quarter = 1", golf ball = 1.75", etc.), estimated wind speed
  6. Duration — how long has the event been occurring
  7. Direction of movement — which way is the storm/event moving

Critical reports (immediate): Tornado, funnel cloud, wall cloud with rotation, wind ≥70 mph, hail ≥1" (quarter), flooding with life safety threat.
Significant reports: Hail <1", wind 50-70 mph, flash flooding, road closures, power outages, structural damage.

Printing Tips

For best results: use Chrome or Edge, select "Save as PDF" or your printer, and choose landscape orientation for ICS-205. Navigation and buttons are automatically hidden when printing. All form fields will print with their entered values.
